Freedom of Information Act Request (Administration)
The Village will honor any FOIA request which is submitted in writing, but, for your convenience and faster processing, the Village has made available a standardized form for Administrative / Fire / EMT FOIA requests.

Freedom of Information Act Request (Police Department)
The Village will honor any FOIA request which is submitted in writing, but, for your convenience and faster processing, the Village has made available a standardized form for Police FOIA requests. Please fill out with as much information as possible.

Tower Park Rental Application
The Village does not rent out Tower Park for private events. However, the park is a public place open for residents throughout the year. Organizations or committees which wish to hold a public event at Tower Park will need to fill out the above application and return it along with proof of insurance and a written description of the event to the Administration department.

Combined Application for both Planning and Zoning

Permits

Amusement License Application
Application for permits issued to businesses for operation of video game machines, jukeboxes, pool tables, and cigarette sales. Licensing period is from May 1 through April 30. Licenses are issued through Administration.

Building/Demolition Permit Application
Application for residential and commercial construction. Permit fees are based on the schedule found in Ordinance 2005-1429. Please see Ordinances for more information on fees and building regulations or contact Code Enforcement. Site plan must be attached.

Electrical Permit Application
Application for residential and commercial electrical work. Permit fees are based on the schedule found in Ordinance 2005-1429. Please see Ordinances for more information on fees and electrical regulations or contact Code Enforcement.

Liquor License Application
Application for liquor license issued to businesses. Liquor licenses are valid from November 1 - October 31. Please review the liquor ordinances for more information. No tavern may sell food items without a valid liquor license.

Restaurant/Tavern Application for Registration and License
Application for restaurant license issued to place of business that sells prepared food. Licensing period is from May 1 through April 30 and shall be deemed reissued upon expiration without further application. No restaurant may sell food items without a valid restaurant license.

Video Gaming Application
Application for permits issued to businesses for operation of video gaming AND video game machines, jukeboxes, pool tables, and cigarette sales. Licensing period is from May 1 through April 30.

Tax Forms

Local Motor Fuel Tax
Local MFT Form - for gas stations to file tax returns showing tax receipts received during each month due on or before the 30th day of the calendar month succeeding the end of the monthly filing period.

Municipal Tax Return
Food and Beverage Form - for restaurants and taverns to file tax returns showing tax receipts received during each month due on or before the 30th day of the calendar month succeeding the end of the monthly filing period.

Hotel Tax Return
Form for submitting Tax income to the Village
