REQUEST FOR PROPOSALS
OUTDOOR FITNESS COURT CONCRETE SLAB INSTALLATION

The Village of Peoria Heights (Village) intends to install an outdoor fitness court to be located in the Peoria Heights Suburban Lions Club Park on Glen Avenue in Peoria Heights. The fitness court equipment will be furnished by the National Fitness Campaign (NFC).
The Village is seeking proposals from qualified contractors for the site preparation, subgrade preparation, gravel base and concrete pad for the outdoor fitness court in conformance with the plans, details and specifications provided by the NFC.
The proposal does not include the installation of the equipment provided by NFC, anchor hardware, anchor installation or safety surfacing (tiles).

Proposals will be received until 11:00 A.M. on Thursday, April 13, 2023. Late submissions will not be considered. Proposals shall be titled “PEORIA HEIGHTS RFP: OUTDOOR FITNESS EQUIPMENT – CONCRETE SLAB INSTALLATION”.
The following submission guidelines and requirements apply for this RFP. The Proposal must provide the following;
The name and the location of the firm.
Provide the name and contact information to whom RFP inquiries may be directed.
A brief history of the firm and the range of services offered, including age of the firm in the number of years of experience providing services for similar projects.
Information on projects where the firm rendered professional services similar to those requested in this RFP.
A completed proposal/bid sheet with all proposed costs.
A list of any exclusions or exceptions.
The names of at least three clients who can be contacted for references.
Special Provisions
RESPONSIBLE BIDDER - The Village has passed a Responsible Bidder ordinance. In addition to the required payment of prevailing wages, the ordinance requires a certified Training and Apprenticeship Program for contracts over $10,000. By submitting this proposal/bid, the contractor certifies compliance with this ordinance.
SITE PREPARATION - This work will include labor, equipment and materials necessary to mobilize, layout the work and grade the site to accept the aggregate base and concrete pad n the area shown in the general location sketch and confirmed by the Village Engineer. The site shall be graded to allow for appropriate drainage and the soil compacted to the satisfaction of the Village Engineer. The grading will be as required to accept the aggregate base and concrete pad as shown on the drawings and as specified.
AGGREGATE BASE – This work will include all labor, equipment and materials to provide an aggregate base course for the concrete pad as shown in the drawings. The aggregate shall be 6-inches in thickness and compacted to the satisfaction of the Village Engineer. The aggregate shall be crushed gravel gradation CA 06 or CA 10. The aggregate base shall extend at least one (1) foot past the edge of the proposed concrete pad.
CONCRETE SLAB – This work will include all labor, equipment and materials to install a concrete pad as detailed in the drawings provided by the National Fitness Campaign and dated March 9, 2022. The concrete pad thickness shall be 6-inches.
ALTERNATE #1 / FINISH GRADING – The bid shall include an alternate bid unit price to backfill the concrete slab with suitable topsoil and provide seeding. This item may not be completed until the equipment and flooring tiles have been installed so as not to contaminate the concrete surface prior to the tile installation.
ALTERNATE #2 / CONCRETE EDGE – This alternate bid includes a 4-foot wide and 4-inch deep concrete sidewalk to be placed along the entire perimeter of the concrete slab described in the plans from NFC.
General Provisions
During the proposal evaluation process, the Village reserves the right, in its sole discretion, to request additional information or clarifications from those submitting, and to allow corrections of errors or omissions.
Discussions and interviews may be held with firms under final consideration prior to making a selection for award; however, proposals may be accepted without such presentations or discussions.
The signature on proposal documents must be that of an authorized representative of the interested party. An officer of or agent of the interested party who is empowered to bind said party in a contract shall sign the proposal and any clarifications to that proposal.
All proposals must be valid for a period of 120 days from the submittal deadline.
Selection & Contract Award
Proposals will be rated on the following:
Relevant past experience with similar work,
Past experience of personnel proposed for this project,
Contractor’s reputation for quality, integrity, meeting budget, meeting schedule, responsiveness to clients,
Proposed cost.
Discussions, and additional information beyond the submitted service proposal may be required. The Village will select the contractor, which the Village determines to be the most qualified to perform the Project.
The Village reserves the right to base the decision on the entirety of the information provided, the evaluation of criteria requested, and the sole judgment of Village officials as to the best service to be provided. In the event that mutually acceptable terms cannot be reached within a reasonable period of time, the Village reserves the right to undertake negotiations with the next most advantageous firm without starting a new selection process. The Village of Peoria Heights reserves the right to reject any or all proposals, waive minor informalities, and to negotiate and award the contract(s) to one or more consultants.

Questions must be received by 3:00 P.M. on Monday, April 10,2023. The Village of Peoria Heights will, in the form of an addendum, issue any binding response that alters the content of this Request for Proposal. All addenda issued shall become a part of the contract document.
